Choosing an out-of-hospital birth with a Licensed Midwife
Inland Empire Midwifery is honored and privileged to serve you and your family. Our responsibility is to support your informed decision-making regarding your birth and care; our practice does not take this lightly. A planned out-of-hospital birth is both your right AND your responsibility as a birthing person. You must deeply feel that out-of-hospital birth and perinatal care provided by a Licensed Midwife is the best choice for you, your baby, and your current life circumstances.
In our culture, midwifery provides an option for parents who believe pregnancy, childbirth, and lactation can be natural and healthy experiences. Focusing on the normal does not mean that problems go unrecognized or unattended. Instead, they provide clues to what needs to be restored. They are neither expected nor feared. If problems occur at home, you will invariably be questioned by friends, family members, and professionals as to the wisdom of your choices. Your midwife asks you to honestly project yourselves into your worst-case scenario and examine how you would feel about your original choices after the fact. We strongly encourage you to have these brave and vulnerable conversations with your midwife so that a trusting relationship can be created, one that recognizes the wonder of birth and the inherent lack of guarantees in both life and birth. Inland Empire Midwifery depends on you, the clients, to stand behind them in case of problems; in the same way, our practice places all agents at personal and political risk so that you may have your choice of care and birthplace.
Emergencies can arise in any birth setting. Some unforeseen events may result in an unexpected outcome. The overwhelming belief in our country is that hospitals are the safest place to give birth. Though the US spends more per capita on medical care than any other industrialized nation, it ranks poorly amongst nations of similar income in life expectancy, infant mortality, and maternal mortality. US ranking in these categories has steadily declined since 1960 (lower rankings=poorer outcomes).
Despite statistical evidence, in choosing a planned out-of-hospital birth, consumers of midwifery care are going against the predominant belief that hospitals are safer, regardless of statistical data. The birth site is seldom questioned when emergencies or poor outcomes occur in a US hospital. However, suppose a similar situation occurs in the community, even if the outcome would have been the same in the hospital; in that case, the choice of a community setting is consistently called into question.
Your midwife cannot guarantee a normal, spontaneous vaginal delivery birth; your midwife cannot guarantee a healthy birthing person or baby; your midwife cannot guarantee an out-of-hospital birth; and acceptance of a client for homebirth in no way constitutes any such guarantee.
At all times, what is GUARANTEED, is compassionate and competent midwifery care.
SERVICE OFFERINGS
Preconception & Fertility Services
This service includes regular assessments of fertility tracking, detailed discussions on lifestyle factors affecting fertility, nutritional guidance, and holistic counseling to optimize preconception health and well-being. We also offer intracervical, intravaginal, and intrauterine inseminations.
Prenatal Care
All prenatal visits occur in our office located in Old Town Temecula. Prenatal care will commence with a comprehensive initial prenatal followed by an in-depth nutritional counseling visit. The typical schedule includes a group or private prenatal visit every two to four weeks thereafter. Private visits average 60 minutes in duration but may be adjusted to suit each family's needs. A prenatal 'Birth Team Meeting' is scheduled at approximately 36 -37 weeks; all birth support people are encouraged to attend this visit.
Postpartum Care
A 24-hour assessment is made over a phone call the following day; your midwife is available anytime by phone during the first week. A home visit is made within 48 hours postpartum and at one week; additional visits are scheduled throughout the first 2-12 weeks. In the event of hospital transport, your midwife or affiliated agents will make phone or hospital visits until you are home. The typical postpartum visit schedule will then be implemented. We encourage you to have household help after the birth; we can provide you with referrals to postpartum doula services upon request.
Home Birth
Inland Empire Midwifery will provide as much or as little labor support as you request, with regular attention, as necessary, to monitor the birthing person and baby. Our best care is based on your clear communication about what you need. If there are any complications, we'll discuss options as the situation unfolds. The midwife's priority is the safety of the birthing person, baby, and others in the birth space. Your birth team will arrive at the birth within a reasonable amount of time after you request our presence and stay an average of four to six hours after the birth to ensure the birthing person and baby are stable.
Newborn Care
Although I will perform routine newborn and postnatal examinations, I cannot provide the same services as a pediatrician. I expect you to interview and choose a pediatrician during your pregnancy and to take the baby for a routine physician evaluation within the first week or sooner if there is an indication. California requires that your baby receive a newborn screening blood test, which we will perform within 48-hours of birth; we will provide you with this information during your pregnancy. Newborns will have routine assessments under our care for the first 6 weeks.
Lactation Services
By feeding your baby human milk, you are providing the absolute BEST start in life! Lactation support by an IBCLC (the goal standard for lactation care and assessment) can be provided for those hoping to induce lactation or wean; hoping start off their breastfeeding/chestfeeding journey off right; or dyads facing feeding struggles.
Well-Person
Inland Empire Midwifery and Lactation also performs well-person preventive care, across the lifespan from teenagers through postmenopausal people! Get your annual exams, paps and STI testing with us!
